Rutgers, The State University of New Jersey is seeking a Habilitation Counselor in Outpatient Treatment within Rutgers, University Behavioral Health Care.
The Habilitation Counselor provides site and community based case management and supported employment services to individuals receiving treatment services through UBHC’s Outpatient Certified Community Behavioral Health Clinic (CCBHC) services.
Among the key duties of this position are the following:
- Supports individuals to gain access to needed medical, social, education, mental health, addiction, co-occurring and other services.
- Links individuals served to needed services including: benefits, medical, dental, mental health, co-occurring services, self-help, employment, legal and housing.
- Assists with scheduling appointments with the individual served and when necessary accompanies the individual served to appointments.
- Provides services that are Person Centered that assure the person’s expressed needs and strength drives the service plan and service delivery.
- Collaborates effectively with members of the department, and with external departments, staff, referral sources, other service providers and social support networks to provide services and rehabilitative care for consumers and families.
