Job Description:
The UTMC Care Clinic Case Manager coordinates the overall interdisciplinary plan of care for patients. They monitor care and act as a liaison between patient/family, healthcare personnel, and insurers. The Case Managers evaluates social determinants of the health for patients and develops, monitors, and updates an individualized service plan. They identify barriers to care and implement revisions to improve health outcomes and quality of care.
Minimum Qualifications:
• Bachelor’s degree in social work (or related field such as public health, psychology, behavioral health, etc.) from an accredited college required.
• Relevant licensure required. (LSW, LISW, etc.)
Preferred Qualifications:
• Experience working with people living with HIV preferred.
• Experience working with diverse populations preferred.
• Master’s degree in social work preferred.
• Bilingual (Spanish/English) preferred.
